R-04 Use headings for real information structure
Headings should communicate a logical hierarchy of topics and decisions. Do not turn them into keyword lists or enforce arbitrary heading-count rules as ranking factors.

Why it matters
Headings are how readers scan a page and how crawlers and answer engines work out which part answers which question. If headings are stuffed with keywords or chosen for their size, people cannot find what they need and machines cannot separate one topic from the next.
Evidence to review
- Heading outline, DOM and accessibility review.
How to implement
- Read the page outline without body copy.
- Use meaningful main and subordinate headings with semantic levels.
- Make each section fulfil its heading.
- Separate visual font size from document structure.
- A heading containing the main term is not automatically stuffing. Preserve headings that help users navigate; do not add one for every synonym.
How UltimateSEO checks this
Automated. A check in the scan sets PASS or FAIL for this control, and reports UNKNOWN or BLOCKED when its input is missing.
The check looks at: a single H1 and no skipped heading levels on crawled pages.
States used: PASS, FAIL, UNKNOWN, NOT_APPLICABLE or BLOCKED. UNKNOWN is never shown as a pass. How states work.
